The Lake County General Health District has announced a vaccination clinic scheduling list. Residents who are 65 and older and/or have certain medical conditions are invited to complete an on-line contact form to be added to the list. You will then be later notified with scheduling instructions when the vaccine schedule has been confirmed. Completing a form does NOT guarantee a vaccine appointment, but will provide you with the best and most up-to-date information possible.
